Passport Information
If you are a Canadian citizen applying for a passport you need to complete the following steps:
1. Download and fill out the neccessary forms from http://www.ppt.gc.ca/cdn/form.aspx?lang=eng®ion=Canada
2. Have proof of Canadian citizenship (Birth Certificate or Certificate of Canadian Citizenship).
3. Have identification supporting your married name.
4. Have 2 identical passport photos taken signed and dated.
5. Pay the required fees.
6. Have any travel documents available if applicable.
